SÍNDROME CARDIOVASCULAR–RIÑÓN–METABOLISMO (CKM): AVANCES EN LA PREVENCIÓN Y EL MANEJO INTEGRAL BASADOS EN EVIDENCIA CIENTÍFICA

El síndrome cardiovascular-riñón-metabolismo (CKM) constituye un marco clínico integral propuesto en 2023 por la American Heart Association. Este enfoque describe cómo interactúan las interconexiones fisiopatológicas entre la acumulación o disfunción del tejido adiposo, los factores metabólicos de riesgo, el daño renal crónico y las afecciones cardíacas. Esta revisión sistemática tuvo como objetivo llevar a cabo un resumen de la literatura científica que ha sido publicada desde el 2021 hasta 2026, en los avances del manejo integral y la prevención del CKM. Para ello, hicimos una búsqueda exhaustiva en bases de datos como PubMed/MEDLINE, Embase, Cochrane Library, Scopus y Web of Science, siguiendo los criterios de la guía PRISMA 2020. Estudiamos guías clínicas, consensos de expertos, ensayos clínicos aleatorizados, metaanálisis y estudios observacionales clave. Clasificar esta patología en cinco fases (de la 0 a la 4) ayuda a estratificar el riesgo y da pie a la intervención clínica desde las primeras etapas. También, las herramientas de evaluación PREVENT ayudan a la predicción de eventos cardiovasculares a los 10 y 30 años al incluir marcadores renales y metabólicos. En el ámbito terapéutico, las opciones con beneficio cardiorrenal particularmente los inhibidores del iSGLT2 y los agonistas del receptor arGLP-1 han demostrado que disminuyen de manera importante los resultados cardiovasculares adversos mayores, la progresión de la insuficiencia renal y la mortalidad cardiovascular. En América Latina y en Ecuador, la alta prevalencia de obesidad, diabetes tipo 2 e hipertensión, aunada a las inequidades del acceso a la salud, complejiza la carga de la enfermedad. En resumen, establecer una estrategia multidisciplinaria que esté abocada hacia hábitos saludables, el seguimiento de indicadores, la prescripción temprana de fármacos en vigilia multiorgánica es el camino más ilustrado para tentar frenar el progreso del CKM e incluso revertir sus estadios.
