LESIONES DEL LIGAMENTO CRUZADO ANTERIOR EN DEPORTISTAS DE ÉLITE: RECONSTRUCCIÓN ANATÓMICA VERSUS TÉCNICAS BIOLÓGICAS

La lesión del ligamento cruzado anterior (LCA) se encuentra entre las causas más comunes de cirugía de la rodilla en el alto rendimiento, teniendo un evidente efecto sobre la carrera deportiva, la estabilidad articular a largo plazo y la reinjuria. La reconstrucción quirúrgica anatómica sin duda es la técnica estándar empleada, pero en los últimos años se ha intentado de forma creciente incorporar técnicas de augmentación biológica (plasma rico en plaquetas, concentrado de aspirado de médula ósea y terapias con células madre) con el objetivo de acelerar la ligamentización del injerto y mejorar el retorno a la alta competición. Evaluar la evidencia científica indexada publicada entre 2016 y 2026 que compara la reconstrucción anatómica aislada del LCA con la reconstrucción con augmentación biológica en los deportistas de élite. Se realizó una revisión bibliográfica de tipo narrativo con apoyo de los lineamientos PRISMA (mediante búsqueda en PubMed/MEDLINE, Scopus, Web of Science, SciELO y Cochrane Library, seleccionando ensayos clínicos, revisiones sistemáticas y metaanálisis publicados en revistas de alto impacto y de referencia). La reconstrucción anatómica continúa siendo la base tratamiento, con tasas de retorno al deporte que rondan el 83% entre los atletas de élite, así como se reducen notablemente las tasas de osteoartrosis postraumática si se respeta la posición anatómica de los túneles. Las técnicas de augmentación biológica en especial el plasma rico en plaquetas y el concentrado de médula ósea demuestran beneficios modestos respecto a la maduración radiológica del injerto y el control del dolor cortoplacista, pero sin traducirse de manera consistente en menores tasas de reruptura ni en un retorno al deporte más temprano. De acuerdo con la evidencia disponible, la reconstrucción anatómica combina bien como la estrategia primera y las técnicas biológicas deben ser contempladas como un complemento individualizado de los principios de biomecánica de la cirugía anatómica, pero no como un reemplazo.
