INTELIGENCIA ARTIFICIAL PARA EL CRIBADO DE RETINOPATÍA DIABÉTICA: PRECISIÓN DIAGNÓSTICA, IMPLEMENTACIÓN EN ENTORNOS REALES Y DESAFÍOS ÉTICOS

La retinopatía diabética (RD) representa la causa principal de ceguera prevenible en las personas adultas en edad laboral, teniendo además una prevalencia en aumento a nivel de la difusión de la diabetes mellitus en el mundo. Objetivo: analizar la evidencia científica publicada desde 2016 hasta 2026 sobre la precisión diagnóstica de la inteligencia artificial (IA) en cuanto a cribado de RD, su aplicación en la práctica clínica real y la ética de su aplicación. Se llevó a cabo una revisión bibliográfica de tipo narrativo con carácter sistemático según lineamientos PRISMA 2020, a través de búsqueda en bases de datos indexadas (PubMed/MEDLINE, Scopus, Web of Science y Cochrane Library) de estudios primarios, revisiones sistemáticas y metaanálisis publicados en revistas de alto índice de impacto. Los algoritmos de aprendizaje profundo alcanzan sensibilidades y especificidades superiores al 90 % en la detección de RP referible bajo condiciones de laboratorio rigurosas; sin embargo, estos algoritmos encuentran sus limitaciones en el nivel del desempeño diagnosticado en poblaciones externas y en particular en países de ingresos bajos/módicos. Los estudios de implementación de la IA en el marco de programas nacionales de cribado documentan aumentos en cobertura y reducción en los costos, aunque las brechas de la generalización aún prevalecen asociadas a la representatividad de las bases de entrenamiento. En la ética, encontramos riesgos de sesgo algorítmico, brechas de equidad, problemas de responsabilidad clínica y consentimiento informado. La IA es vista como una herramienta con potencial para aumentar el acceso al cribado de RD, aunque su utilización requiere rigurosos estudios externos que validen su funcionamiento, la creación de marcos regulatorios claros y la implementación de estrategias para mitigar sesgos.
