RESISTENCIA A LA INSULINA EN LA DIABETES MELLITUS TIPO 2 Y SU PAPEL EN LA PROGRESIÓN DE LA ENFERMEDAD RENAL CRÓNICA Y LAS COMPLICACIONES CARDIOVASCULARES: UNA REVISIÓN NARRATIVA CRÍTICA CON BÚSQUEDA ESTRUCTURADA

Resumen

La diabetes tipo 2 es un problema de salud que afecta a muchas personas en todo el mundo. Sus complicaciones, como problemas en los riñones y el corazón, provocan muchas muertes y enfermedades. La resistencia a la insulina es un factor clave en la diabetes tipo 2. Durante mucho tiempo se creyó que esta resistencia solo afectaba el control de la glucosa en la sangre. Pero en los últimos años se ha empezado a estudiar su posible papel como conexión entre el daño en los riñones y en los vasos sanguíneos. El objetivo de esta revisión fue analizar la evidencia disponible sobre cómo la resistencia a la insulina se relaciona con la progresión de la enfermedad renal crónica y con complicaciones cardiovasculares en adultos con diabetes tipo 2. Se buscó información en bases de datos como PubMed, Scopus, Web of Science, ScienceDirect y Cochrane Library entre enero de 2020 y agosto de 2026. Se incluyeron estudios observacionales, revisiones sistemáticas, ensayos clínicos y guías. Los estudios analizaban la resistencia a la insulina y su relación con problemas renales como la albuminuria, la reducción de la tasa de filtración glomerular y la enfermedad renal terminal. También se estudió su conexión con problemas cardiovasculares como eventos graves o insuficiencia cardíaca. Los resultados muestran que la resistencia a la insulina está relacionada con la presencia de albumina en la orina y con una peor función renal. Esto ocurre probablemente debido a factores como la hiperfiltración, la inflamación, el estrés oxidativo y la lipotoxicidad. Además, hay una clara conexión entre la resistencia a la insulina y la aterosclerosis, el daño en las arterias y los problemas cardíacos. Sin embargo, no hay mucha evidencia directa que demuestre cómo la resistencia a la insulina lleva al inicio de la hemodiálisis. Algunos medicamentos, como los inhibidores de SGLT2, los agonistas del GLP-1 y la finerenona, ofrecen beneficios que van más allá del control de la glucosa. Se concluye que la evidencia apoya una relación clara y lógica entre la resistencia a la insulina, el daño renal y el riesgo cardiovascular. Sin embargo, las diferencias entre los estudios y el hecho de que la mayoría sean observacionales limitan la posibilidad de afirmar causas directas.
